I've had good results from them so far and they are especially cheap
for me since I'm in Kuala Lumpur and can simply pick up my PCB by
driving to their office ;-) You'll have to factor in shipping cost.
These guys specialise in prototyping to the extent that they max out
their production capacity at 500 pieces (1000 if single sided).
Minimum quantity is 2.

Yes, that's what we do.  We design boards to customer's spec.

Actually, we prefer QFN over TQFP.  Smaller chip will have lower
product cost.
They are tough to prototype, but assembly houses don't care.  Just
treat the QFN as ball-less BGA with outer rows only.  Unlike BGA, QFN
can be inspected and reworked manually as well.

hardware-wise, how do I go about getting it produced?  The AVR 
microcontroller I'm using (AT90USB1287) is way too small for me to 
manually build a board.  Are there places that will build a board to my 
specs in low volumes and affordable prices?  Or am I going to be 
limited in my choices to ICs with old-school packaging?

Any suggestions would be a great help, as this is yet another area I'm 
entering that I have no experience in.
