Hi,
I have the same problem Whitch ICD2 and 16F628.
I use the ICD2 for debugger. And any PIC's have ICD suppurt. 16F628 have not ICD suport. But 16F628A have support. you can use it for debug.
 
For Programing Chip use a Programmer. I use Micropro PIC Programmer (Diyk149B kit).
I have make it by myself and it work perfect.

you sure it supports the 628? I thought it supported the 628A but not
the non-A part.
