Take a look at the power supplies. I had a similar problem with the
2119 and traced it to needing a larger bypass cap on the VCC pin.
Looking at the pin with a meter, everything was fine. I had to go to
100MHz view, AC coupled, then saw major spikes in power supply. The
device "kind of worked" - I could do some JTAG, some ISP. Adding a
10uF
chip cap at the regulator cleared it all up.

I have a LPC2138 soldered onto my own board. I have a USB-serial
bridge IC attached to UART0. I am using a 12MHz crystal (I have also
tryed 16MHz) I have no 32kHz crystal attached.
Using Phillips flash utility I can read device ID: 196389, Boot loader
ID: 2.0.
I can successfully erase and blank check. However when I attempt to
Upload Flash it fails mid way through the write. I have tryed using
all the baud rates. Any ideas?
